RN Case Manager Hospice

Job Title: PRN Registered Nurse – Hospice

Employment Type: PRN (As Needed)

Position Summary

We are seeking a compassionate and experienced PRN Hospice Registered Nurse (RN) to provide high-quality end-of-life care to patients in their homes. This role focuses on delivering skilled nursing care, supporting families, and ensuring comfort and dignity for patients during their final stages of life.

Essential Responsibilities

· Provide skilled nursing care to hospice patients in accordance with the individualized plan of care.

· Perform comprehensive patient assessments and monitor changes in condition.

· Administer medications and treatments as prescribed by the physician.

· Educate patients and families on symptom management and hospice philosophy.

· Collaborate with the interdisciplinary team to ensure coordinated care.

· Maintain accurate and timely documentation in compliance with regulatory standards.

· Respond to urgent patient needs and provide crisis intervention when necessary.

Qualifications

· Current Texas RN license.

· Minimum of 1 year of nursing experience (Hospice or Home Health preferred).

· Current CPR certification.

· Reliable transportation and valid auto insurance.

· Strong communication and interpersonal skills.

· Proficiency in EMR systems (experience with Kinnser preferred).

Schedule

PRN (Flexible scheduling based on patient needs)

Travel required within Tarrant County
