Eliot is seeking an energetic and empathic Health Home Care Manager to support our behavioral health clients in medical and mental health needs. Eliot's Health Home program is an innovative care management approach for adults with complex medical, behavioral health, and social needs. The ideal candidate for this position will have a strong commitment to mental health recovery, knowledge of substance abuse disorders and commitment to coordination and integration of all aspects of health care. Additionally, the candidate should possess strong time-management, organizational and communication skills.
Responsibilities:
- Coordinate care with the member and their medical, health, substance use, mental health and other related providers to ensure integrated treatment and care
- Provide and coordinate post hospital discharge follow-up, as needed
- Maintain contact with payer systems communicating and documenting coordination activities and service authorizations
- Provide initial and on-going member engagement, support and care coordination.
- Partner with the member to develop and implement a person-centered care plan.
- Connect members with community services and resources
- Convene interdisciplinary care team and coordinate activities to ensure integration
- Promote hope and empowerment through meetings and support with members
- Facilitate access to transportation/ mobility resources that enable members to fully participate in medical services, behavioral health services and community resources
- Ensure that clients are treated with dignity and respect in accordance with ECHS' policy
Qualifications:
- Requires a Bachelors degree in Psychology, Social Work or related field
- Experience working in care management or case management for adults
- Valid Massachusetts drivers license
Schedule: Monday-Friday, 9am-5pm
