Job Summary

The Advanced Practice Registered Nurse (APRN) is responsible for providing medical services for the patients of Heart of Florida Health Center.  The APRN will work collaboratively with physicians and other healthcare professionals to deliver comprehensive primary care services to Patients in a Correctional Facility.

Qualifications and Requirements

  • Master's or Doctoral degree from an accredited Nurse Practitioner program.
  • Florida state licensure as an Advanced Practice Registered Nurse (APRN).
  • National certification in your NP specialty area through an accredited certifying body (e.g. ANCC, AANP).
  • Maintain continuing education requirements to renew licensure and certification periodically.
  • Complete credentialing process as required by Health Resources and Services Administration (HRSA).
  • Strong clinical skills and knowledge of primary care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and collaborate with a multidisciplinary team.
  • Proficiency in using electronic medical records (EMR) and medical software.
  • At least (2) two years experience working in a Correctional or lock-down facility.

Essential Functions

  • Represent HFHC values and mission.
  • Knowledgeable about the HFHC philosophies, aims, and goals.  Be aware of and abide by all HFHC policies and procedures and all local, state, and federal laws.
  • Maintain the highest standards of medical practice, ethics, and professionalism.
  • Provide comprehensive primary care services, including acute and chronic care and preventative and health maintenance in a correctional setting.
  • Follow current disease management protocols and clinical guidelines outlines in HFHC policy to enhance clinical performance.
  • Conduct physical assessments using observation, inspection, auscultation, palpation, and percussion; order/test, analyze, and interpret diagnostic images to evaluate the patient's condition and prescribe medications and therapies for treatment and preventative care.
  • Ensure appropriate medical management of acute and chronic conditions as indicated by outcomes on Quality Measures Performance, and other relevant reports (peer reviews, any focused reviews, and patient satisfaction surveys).  Ensure timely completion of required documentation and assigned tasks according to HFHC policies and procedures, including documents, progress notes, telephone encounters, labs, medication refills, referrals, and other necessary tasks.
  • Report relevant information or situation affecting patient care or HFHC liability to the Medical Director and/or CMO as necessary.
  • Serve as a resource for medical and operations support staff.
  • Complete all required training courses on time and maintain proper certifications.
  • Participate in Peer Review and Quality Assurance activities as requested.
  • Always maintain a proper and professional appearance.
  • Submit necessary information to the Health Department in accordance with state law regarding communicable diseases.
  • Perform other duties as assigned.
  • Duties demand attention to detail, alertness, problem solving, stress tolerance, and sound judgement.

Physical Requirements

  • Frequently sitting at a desk for office work.
  • Occasional driving for external appointments/meetings.
  • Requires continuous use of corrected vision, depth perception, wide field of vision, and color recognition. The role also involves ongoing use of olfactory, auditory, and tactile senses.
  • Position is highly active and requires standing, walking, bending, kneeling, and stooping throughout the day.
  • Frequently lift and move items weighing over 30 pounds, including equipment, supplies, and other materials required for the job.
