Job description

We are looking for a Case Manager (Bilingual preferred) to oversee a caseload of approximately 35-40 clients and join our team in San Antonio. Our agency serves Individuals with Intellectual Developmental Disabilities (IDD) in Medicaid Waiver programs, such as Home and Community-Based Services (HCS); Texas Home Living (TxHmL); and Community Living Assistance and Support Services (CLASS).

Case Managers perform several tasks to help clients with IDD live and prosper in various community settings. The Case Manager will work directly with clients, family members, and providers, as well as other health care professionals and agencies, to assist in coordinating goal-oriented plans of care for the individual.

Responsibilities include:

  • Develop, monitor, and evaluate person-centered plans of care and progress in accordance with professional standards and HCS/ TXHML/CLASS program guidelines; help clients achieve wellness and autonomy
  • Advocate for and facilitate referrals to community resources. Coordinate multiple services, including residential options, transportation, day programming, and therapies, such as behavioral supports, PT/OT, and speech therapy
  • Maintain clients' records by completing home visits, reviewing case notes, logging events and monitoring quarterly progress; record and document accurately on necessary forms
  • Help clients make informed decisions by acting as their advocate regarding their person-centered plans and treatment options, financial benefits and medical insurance
  • Develop effective working relations; teamwork oriented to promote quality interventions and outcomes; work closely with other health care professionals; intervene in crises and provide support to clients
  • Orient and train providers of clients to follow plans of care, documentation requirements and achieve outcomes in plans of care

Case Manager Skills and Qualifications:

  • Compassionate with ability to relate and connect with IDD clients with various needs
  • Strong time management, organizational, and documentation skills needed to maintain multiple case records simultaneously
  • Strong verbal and written communication skills to explain to clients, family members, and professionals the plans of care
  • Critical thinking and problem-solving skills; ability to collaborate effectively with others to achieve common goals
  • Motivated to research and analyze information to properly address the needs of a client
  • Familiar with professional standards and emerging technical knowledge

Education and Experience:

  • High School Diploma required; Bachelor's Degree preferred (health related/social services preferred but not required), or equivalent combination of education, training, and experience
  • Professional or personal experience supporting individuals with disabilities, mental and/or behavioral health challenges
  • Must have a valid driver's license; provide current registration and insurance; and be able to pass a background check

Work Hours & Benefits

Full-time schedule (40 hours a week) with normal business hours Monday through Friday (8:30am to 5:30pm) and available flex time when fieldwork requires late or early appointments. Company vehicles available for face-to-face meetings with clients. Health, Vision, and Dental benefits available to full-time employees.
