About

The Scheduling & Intake Coordinator plays a key role in ensuring smooth operations within our Home Health and Hospice programs. This position manages all new patient referrals, verifies insurance, gathers required documentation, and coordinates scheduling with clinicians and patients. The ideal candidate is organized, compassionate, and able to thrive in a fast-paced healthcare environment.

Key Responsibilities

Intake & Referral Management

  • Receive and process incoming referrals from hospitals, physicians, facilities, and families.
  • Communicate with referral sources to obtain necessary documents (orders, face-to-face, demographics, clinical notes).
  • Verify insurance eligibility, benefits, and authorization requirements.
  • Enter referral and patient data accurately into the EMR system.
  • Ensure all admission requirements are complete before scheduling.

Scheduling & Coordination

  • Create and maintain accurate schedules for nurses, therapists, aides, and other clinicians.
  • Assign appropriate staff based on patient needs, location, and availability.
  • Communicate schedule changes promptly to clinicians and patients.
  • Monitor visit frequencies, ensuring timely follow-ups and compliance with care plans.

Communication & Support

  • Serve as a primary point of contact for patients, families, clinicians, and referral partners.
  • Answer incoming calls regarding scheduling, service requests, and general inquiries.
  • Collaborate with clinical and administrative teams to ensure smooth workflow and high-quality care.

Documentation & Compliance

  • Maintain accurate, compliant, and up-to-date patient information.
  • Follow HIPAA guidelines and all agency policies.
  • Assist with reporting, patient charts, and documentation audits as needed.

Qualifications

  • Open to fresh graduates
  • High school diploma or equivalent required; associate degree preferred.
  • Spanish expertise preferred
  • Experience in home health, hospice, healthcare scheduling, medical office, or similar field preferred.
  • Strong organizational and multi-tasking ability.
  • Excellent communication and customer service skills.
  • Proficient with computer systems and EMR/EHR software.
  • Ability to work in a fast-paced environment with urgency and accuracy.
