JOB SUMMARY
This position assures appropriate data collection, abstraction, analysis and reporting of noted measures and communicates changes in definitions, methodology, and mandatory requirements. This position will support the quality and patient safety improvement efforts through data abstraction and inputting into the appropriate system.
ESSENTIAL JOB FUNCTIONS
Performs clinical data abstraction, valid analysis of data and reporting of hospital clinical quality of care performance information to regulatory agencies by established guidelines.
Identifies issues of concern, emerging trends, and areas of interdepartmental need and processes requiring action to adequately adapt to the dynamics impacting quality of care.
Works closely with medical, nursing, information systems, informatics, and hospital clinical staff to improve adherence with quality indicators through education.
Identifies strategies to attain quality improvement goals.
Keeps abreast of National, State and SMH patient safety goals, objectives and best practice guidelines and practices.
Presents patterns and trends and/or variance analysis to appropriate committees.
Working knowledge of EMR, CMS, Joint Commission and other regulatory entities.
Serve as a subject matter expert to clinical quality measures within the organization.
Coordinates updates, EMR package upgrades and measure readiness across teams within the organization.
Performs audits to validate accuracy, completeness and compliance with measure requirements.
Ensures compliance and timely submission with all necessary reporting outcomes for organization.
Contributes to performance improvement projects by identifying gaps, recommending process improvement initiatives and tracking outcomes.
Develops standard operating procedures for abstraction processes, measure review and validation.
Participates in efforts to optimize EMR build and workflows to improve data reliability and measure performance.
POSITION QUALIFICATIONS
Education, Experience & License
Bachelor's degree in Nursing, required.
Licensed Registered Nurse in the State of Wyoming.
BLS, preferred.
Minimum of 5 years nursing experience, required. Acute care nursing experience, preferred.
Minimum of 2 years healthcare quality, clinical data abstraction, medical record review, or performance improvement experience, preferred.
Certified Professional in Healthcare Quality (CPHQ), Certified Health Data Analyst (CHDA) or Certified Professional in Healthcare Information & Management Systems (CPHIMS), preferred.
Familiarity with electronic medical record navigation and experience with data abstraction, strongly preferred.
Additional Skills
Knowledge and understanding of process improvement, theory/tools.
Knowledge and understanding of TJC standards. Understanding of CMS and all other regulatory requirements.
Knowledge of performance improvement principles, tools and techniques
Ability to take initiative and work with minimum supervision
Must be proficient in the use of Microsoft Office applications including Excel, Word, and Power Point and Outlook.
Must possess sound oral and written communication skills including clear articulation of ideas, proper grammar and spelling and poise and professional appearance.
Attention to detail with follow-up is necessary.
Specific demands not listed: Possible exposure to blood and or body fluids / infectious disease / hazardous waste requiring the use of Personal Protective Equipment. Exposure to odorous chemicals / specimens and Latex products.
