Position Details
The Family Based Case Manager/Mental Health Counselor provides comprehensive case management and connects families to community programs and services. This role includes co-facilitating family therapy sessions, delivering parent coaching, and providing crisis intervention for children assigned to the caseload. Clinical sessions are primarily conducted in the community, including client homes and schools, while case management activities may occur in an office or community setting. The position also requires on-call coverage, offering flexibility and responsiveness to meet client needs.
