*Responsibilities*
* Conduct comprehensive patient assessments to determine physical therapy needs across inpatient, outpatient, pediatric, geriatric, and home health populations
* Develop and implement tailored care plans that incorporate evidence-based practices such as trigger point therapy, orthopedics techniques, and physiotherapy modalities
* Perform therapeutic interventions including manual therapy, exercise prescription, and mobility training to promote recovery and functional independence
* Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions between care settings
* Maintain accurate documentation of patient progress within Electronic Medical Record (EMR) systems and adhere to medical terminology standards
* Educate patients and caregivers on injury prevention, physiologic processes, and home exercise programs to support ongoing recovery
* Stay current with advances in physiology knowledge, anatomy understanding, and outpatient care practices to deliver optimal patient care
*Skills*
* Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice
* Proven experience in acute care hospital environments, outpatient clinics, or home & community care settings
* Skilled in discharge planning and creating effective care plans for diverse populations including pediatrics and geriatrics
* Proficiency with EMR systems for documentation and patient management
* Expertise in inpatient rehabilitation, outpatient therapy services, and home health programs
* Familiarity with trigger point therapy techniques and orthopedic rehabilitation methods
* Excellent communication skills to educate patients effectively and collaborate with healthcare teams
