Job Summary
The Care Coordinator is accountable and responsible for evaluating client needs, overseeing and monitoring the care delivered, developing an appropriate plan of care, and ensuring the competency of caregivers.   The Care Coordinator commits to participate in establishing and implementing the highest standards of care.   
The Care Coordinator is responsible for the quality and adequacy of services provided and will provide leadership and direction in all aspects of client care.   She/he functions and participates in the development, implementation, monitoring, and evaluation of client programs and services while upholding Qualicare brand standards.   This leader will assist in the delivery of home care services including client assessments, health education/counseling, and also will provide some direct client care.   
Responsibilities
  • Assist business development efforts in the territory by accompanying the GM(s) to various events, speaking engagements and other community initiatives
  • Consults with supervisors and staff regarding clients, families and service programs
  • Conducts in-home assessments and creates care plans for clients
  • Consults with physicians and other care providers related to client care services
  • Performs supervisory visits to clients on a regular basis
  • Prepares, administers and appropriately documents actions taken specific to medications
  • Performs all skills, treatments, and procedures competently in accordance with the provincial scope of practice
  • Participates in employee selection process and supervises the overall performance of field teams
  • Provides backfill resources as needed for team
Qualifications
  • Registered Nurse or other regulated health professional designation (RPN, LPN, Occupational Therapist, Social Worker, etc.) with 2+ years of clinical experience.
  • Strong clinical skills
  • Strong communication skills; ability to interpret clinical information and explain care plans to clients and families.
  • Existing relationships in hospitals or community health networks - business development is everyone's responsibility
  • Negative TB test or chest x-ray
  • Certified in CPR
  • Self-directing with the ability to work with little direct supervision
  • Excellent customer service skills
