Position Summary

We are seeking a passionate and dedicated Targeted Case Manager to join our growing team. In this vital role, you will work directly with individuals in recovery, helping them navigate barriers and connect to essential resources. You'll be part of a multidisciplinary team working together to create a supportive environment that empowers each client on their recovery journey.

Key Responsibilities

  • Conduct initial and ongoing assessments to identify client needs, strengths, and recovery goals
  • Create and manage individualized case management treatment plans
  • Connect clients with critical services such as medical care, housing, employment, and more
  • Monitor progress and adjust care plans to ensure continued growth and engagement
  • Provide crisis intervention and de-escalation support when needed
  • Advocate for clients with external providers and social service systems
  • Collaborate with therapists, medical staff, and other team members for integrated care
  • Maintain thorough, timely documentation in compliance with all regulations
  • Promote a supportive, non-judgmental environment that encourages hope and resilience
  • Uphold ethical standards and ensure client confidentiality at all times

Qualifications

  • Must meet Kentucky Targeted Case Management (TCM) qualifications:
  • Bachelor's degree in a human services field plus one year of related experience post degree OR
  • Master's degree in a qualifying field
  • Experience working with individuals with substance use and co-occurring disorders preferred
  • Strong communication, organization, and problem-solving skills
  • Knowledge of local community resources is a plus

Job Type: Full-time

Pay: $42, $48,000.00 per year

Application Question(s):

  • The first position consists of a 9am-5:30pm schedule for a PHP LOC at our primary clinical building in south Louisville. The second position consists of a 11-7pm four weekdays a week and one weekend day 8-4:30pm at our OP LOC.
