Join our dedicated team as a Hospice RN Case Manager (RNCM)

Are you passionate about providing compassionate care to patients at the end of life? AdvisaCare is seeking an experienced Hospice RNCM to manage patient care and ensure our patients receive the highest quality of support in Oakland County. In this role, you will coordinate and oversee patient care, working closely with our multidisciplinary team to create personalized care plans that meet the unique needs of each patient and their family.

Your responsibilities will include conducting assessments, coordinating services, providing guidance and education to patients and families, and ensuring compliance with all regulatory requirements.

Requirements

  • Current Michigan RN License in good standing
  • Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care preferred
  • Strong communication and interpersonal skills
  • Ability to work collaboratively within a team for optimal patient outcomes
  • Compassionate and empathetic care approach
  • Strong organizational and time management skills
