About
La Vita Bella Home Health and Hospice is seeking compassionate and dedicated Speech-Language Pathologists to provide high-quality, patient-centered care to individuals in the comfort of their homes.
In this role, you will evaluate, diagnose, and treat patients with speech, language, cognitive, communication, and swallowing disorders. You will develop and coordinate individualized treatment plans, collaborate with physicians and interdisciplinary team members, and support patients in improving functional communication, safety, and overall quality of life. You will also educate patients and their families, monitor progress, and provide clinical support to help patients safely manage their care at home.

*Qualifications:*
* A master's or doctoral degree in Speech Language Pathology.
* Licensed Speech Language Pathologist in the state of New Mexico.
* Successfully completed 350 hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience.
* Two years of experience (preferred)
*Responsibilities:*
* Improves or maximizes the communication of the patient.
* Returns the individual to optimum and productive living within the patient's capabilities.
* Provides full range Speech Language Pathology Services as ordered by physician.
* Performs all skilled procedures.
* Instructs patients and family members in home programs.
* Participates in bi-weekly skilled clinical meetings.
* An initial evaluation, including plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by physician every 60 days is necessary if further treatment is to be continued.
* Assists the physician in evaluating in level of function.
* Prepares clinical and progress notes.
* Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
*Lifting Requirements:*
Ability to perform the following tasks if necessary:
* Ability to participate in physical activity.
* Ability to work for extended period of time while standing and being involved in physical activity.
* Moderate lifting.
* Ability to do extensive bending, lifting and standing on a regular basis.
Pay: $77,571.00 - $109,512.00 per year
