Summary:
We Level Up Treatment Program in New Jersey is seeking a Case Manager. The Case Manager is responsible for managing all aspects of the patient's care, treatment, and services throughout the continuum of care.
Job Requirements:
- Case manager will ensure that the patient is safely transitioned from admission, treatment, discharge planning and aftercare in a manner that ensures access to the best and most effective treatment and care alternatives.
- Services include, but are not limited to, biopsychosocial assessment, interdisciplinary treatment planning, participating in interdisciplinary treatment team, coordinating family outreach and family Zoom sessions, social services support, discharge planning, and aftercare follow up.
- Interventions may include return to work or school coordination, applications for programs or services, assistance with completing FMLA or disability forms, coordination of follow up services, and interaction with governmental agencies when required.
- Position requires concurrent chart audits for timely and effective documentation as well as coordination with utilization review departments for insurance reviews.
Educational Background and experience:
- Minimum bachelor's degree in social work, social services, psychology, nursing, or other mental health related field. Individuals with a master's degree will be given priority. In lieu of a degree, three or more years of relevant experience will be considered.
- Credentialed or Professional License relevant to degree or field of study will be given priority; credentialing will be encouraged.
Computer Equipment and Software Requirements:
- Microsoft Office
- KIPU EMR (preferred)

Pay: $50, $55,000.00 per year
