The Case Manager plays a vital role at SACHR in supporting people who use drugs and other marginalized individuals by providing compassionate, participant-centered services grounded in harm reduction principles. The Case Manager works closely with participants to assess needs, reduce barriers to care, and support their goals related to health, housing, substance use, and overall well-being. This position emphasizes relationship-building, advocacy, and meeting people where they are without judgment.
Responsibilities
- Conduct comprehensive intakes and needs assessments with participants.
- Develop individualized service plans that reflect participant goals and priorities.
- Provide ongoing case management, including referrals and linkages to housing, healthcare, substance use treatment (including MAT), mental health services, and social supports.
- Assist participants with navigating public benefits, entitlements, and other resources (e.g., Medicaid, SNAP, SSI).
- Maintain regular contact with participants through in-person meetings, phone calls, and outreach.
- Support harm reduction services such as syringe access, overdose prevention education, and safer use supplies.
- Collaborate with interdisciplinary team members including peers, nurses, outreach workers, and behavioral health providers.
- Advocate for participant needs with external service providers and systems.
- Document all services and interactions accurately and timely in agency databases and case files.
- Participate in case conferences, supervision, and staff development trainings.
- Uphold trauma-informed, anti-racist, and person-centered care approaches in all interactions.
- Perform other duties as assigned.
Qualifications
Required
- Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field; or equivalent relevant experience.
- Understanding of and commitment to harm reduction, social justice, and health equity.
- Strong communication, organizational, and problem-solving skills.
- Experience working with people who use drugs, those experiencing homelessness, LGBTQ+ individuals, or people living with HIV/HCV.
Preferred
- Availability 6am-2pm
- Bilingual (e.g., English/Spanish).
- Knowledge of local Bronx and NYC resources, housing systems, and public benefits.
- Familiarity with case management practices and documentation standards.
- Experience using client database systems (e.g., AIRS, eSHARE, MDToolBox).
