Position Summary
The Lead Case Manager is a pivotal leadership role responsible for organizing, monitoring, and ensuring the delivery of high-quality services within the GGCRN Street Outreach program. This position bridges administrative oversight with field-based intervention to reduce gun violence and promote healing in the Greater Grand Crossing community.
The Lead Case Manager provides critical supervision and technical support to the Outreach Worker (OW) team, ensuring all data, interventions, and case management activities align with mandated deliverables and trauma-informed best practices.
Key Responsibilities
1. Program Oversight & Data Management (30%)
- Technical Proficiency: Maintain and audit all participant data using Salesforce and SharePoint. Ensure all data entry is accurate, timely, and compliant with contract targets.
- Reporting: Provide weekly reports to the Program Manager on new and existing cases. Use Microsoft Outlook for all internal and external professional coordination.
- Audit Readiness: Monitor, review, and maintain records to ensure the program is prepared for audits and contract reviews at all times.
2. Clinical Assessment & Case Management (30%)
- Risk Assessments: Conduct intake assessments to evaluate participant risk and resilience; repeat assessments every 3 months to track progress.
- Caseload Management: Maintain a personal caseload of 10 high-risk participants, ensuring a minimum of 20 hours of contact per month.
- Wrap-Around Services: Identify and refer participants to essential services including mental health care, housing, food assistance, employment, and funeral planning.
3. Team Leadership & Field Support (25%)
- Staff Mentorship: Assist Outreach Workers in implementing the "Scope of Work," providing guidance on crisis intervention, mediation, and problem-solving approaches.
- Emergency Response: Work with the Emergency Response Team to address incidents of violence, promote de-escalation, and reduce the risk of retaliation.
- Direct Advocacy: Assist participants and their families in navigating system partners, including court visits and trials.
4. Community & System Collaboration (15%)
- Rapport Building: Develop and maintain a community resource database to connect participants to educational and career opportunities.
- Interagency Representation: Attend weekly Street Outreach and Violence Interruption meetings, as well as agency-wide and community-based convenings.
Skills & Qualifications
- Software Skills: High proficiency in Salesforce, Microsoft Outlook, and Excel is required for tracking statistics and reporting.
- Crisis Intervention: Ability to utilize restorative justice and mediation techniques in high-pressure, high-risk environments.
- Trauma-Informed Care: Ability to provide trauma-informed support to victims, families, and staff members (Outreach Workers) facing vicarious trauma.
- Physical Stamina: Must meet the physical requirements for field-based work, including navigating diverse urban environments in various weather conditions.
Working Conditions
- Hybrid Environment: A mix of administrative office duties and field-based outreach work.
- On-Call/Flexibility: Must be willing to arrange work hours to benefit the program, including responding to emergency alerts and attending community meetings outside of standard business hours.
- Professional Standards: Conduct all functions in accordance with Agency guidelines, ethical standards, and mandated reporter requirements.
Mandatory Requirements
- Valid Driver's License: Must possess and maintain an Illinois driver's license in good standing.
- Reliable Vehicle: Possession of a working insured personal vehicle is mandatory for field-based work and emergency response.
- Education: Bachelor's degree in a related field and 2+ years of experience in street outreach or violence prevention. (Master's in Social Work or Public Health strongly preferred).
