About

Key Responsibilities

  • Identify and meet patient goals through holistic assessment and functional reassessments.
  • Conduct thorough evaluations using standardized procedures and point-of-service documentation.
  • Develop and implement individualized, goal-oriented treatment plans.
  • Document treatment progress and discharge summaries with accuracy and clarity.
  • Collaborate with nursing staff, residents, and families to maximize patient outcomes.
  • Provide weekend support as needed to ensure continuity of care and compliance with Medicare standards.
  • Deliver evidence-based, patient-centered therapy that promotes independence and quality of life.
  • Educate residents, families, and caregivers on therapeutic programs and home exercises.
  • Participate in interdisciplinary team meetings and provide input on resident progress and care needs.
  • Represent the rehabilitation team in facility meetings and collaborate with marketing/sales teams as required.
  • Ensure residents are fully informed about services, procedures, and any associated risks, documenting all communication in the EMR.
  • Maintain productivity levels consistent with facility expectations.

Qualifications & Skills

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Valid, active state license to practice Speech-Language Pathology.
  • Clinical experience as an SLP (CFY candidate considered)
  • Knowledge of current treatment practices and evidence-based interventions.
  • Familiarity with OBRA regulations, HIPAA, LCD, and third-party reimbursement guidelines.
  • Compliance with ASHA standards and all state/national regulatory requirements.
  • Strong interpersonal, verbal, and written communication skills.
  • Professionalism, flexibility, and a commitment to continuous learning.
