About
Key Responsibilities
- Identify and meet patient goals through holistic assessment and functional reassessments.
- Conduct thorough evaluations using standardized procedures and point-of-service documentation.
- Develop and implement individualized, goal-oriented treatment plans.
- Document treatment progress and discharge summaries with accuracy and clarity.
- Collaborate with nursing staff, residents, and families to maximize patient outcomes.
- Provide weekend support as needed to ensure continuity of care and compliance with Medicare standards.
- Deliver evidence-based, patient-centered therapy that promotes independence and quality of life.
- Educate residents, families, and caregivers on therapeutic programs and home exercises.
- Participate in interdisciplinary team meetings and provide input on resident progress and care needs.
- Represent the rehabilitation team in facility meetings and collaborate with marketing/sales teams as required.
- Ensure residents are fully informed about services, procedures, and any associated risks, documenting all communication in the EMR.
- Maintain productivity levels consistent with facility expectations.
Qualifications & Skills
- Master's degree in Speech-Language Pathology from an accredited program.
- Valid, active state license to practice Speech-Language Pathology.
- Clinical experience as an SLP (CFY candidate considered)
- Knowledge of current treatment practices and evidence-based interventions.
- Familiarity with OBRA regulations, HIPAA, LCD, and third-party reimbursement guidelines.
- Compliance with ASHA standards and all state/national regulatory requirements.
- Strong interpersonal, verbal, and written communication skills.
- Professionalism, flexibility, and a commitment to continuous learning.
