Speech Language Pathologist

Location: Suffolk County, Long Island | Per Diem | Non-Profit Healthcare

As a Speech Language Pathologist (per diem), you'll work on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.

The Speech Language Pathologist (per diem) will provide rehabilitative services to a variety of home care patients.

Provides skilled speech - language therapy services to VNSHS patients.

Use of Chronic Disease management and health promotion strategies.

Use of Motivational Interviewing Techniques.

Provides initial and ongoing speech - language assessment and intervention, in accordance with currently approved methods and standards of practice.

Initial evaluation focuses on the assessment of speech, language, cognition and swallowing disorders.

POC focus on patient centered goals.

Clear documentation of skilled interventions and patient's progress towards or revision of goals as indicated.

Provides case-management oversight and case coordination as necessary when other disciplines have discontinued their plan of care.

Identifies patient centered goals and an interdisciplinary team approach to meet these goals, under the direction of the home care physician, with the most efficient use of resources and avoidance of re-hospitalization.

Interacts and is prepared to discuss his/ her entire case load to include patient's reason for home care and progress towards patient centered goals; follows up on Care manager and / or Interdisciplinary group recommendations.

Completion of HHA supervisions when indicated.

Provides mandated federal and state notices to patients to include, but not limited to, NYS Provision of Care, Home Health Beneficiary notice and Notice of Medicare Non-Coverage.

Maintains and collaborates with other rehabilitation therapists caring for the same patient to ensure timely documentation and assessment of patient progress towards goals as mandated by CMS.

New York State License, Certificate of Clinical Competence and current registration as a language pathologist

Able to comply with the New York State regulatory requirements to provide patient care

Able to comply with all VNSHS policies, procedures and standards of conduct

Previous experience as a language pathologist
