Case Manager

Twin Rivers Developmental Supports, Inc., is currently accepting applications for a Case Manager. The Case Manager is responsible for counseling and coordinating services for individuals on an assigned case load. This is a full-time position working 8:00 4:30 p.m. Monday through Friday.

Job Duties:

• Obtain and maintain a license with Health Care Policy to provide Case Management.
• Write and implement plans and programs.
• Serves as contact for parents/guardians and other professionals.
• Abide by the Health Care Policy Rules of Conduct for Case Managers.
• Assist with securing needed support and ancillary services.
• May be required to assist as back up, fill in and behavior management.
• Transporting individuals to and from appointments as needed.
• General office duties.
• Other duties as assigned.

Qualifications:

• Bachelor's degree (full-time experience in the field of developmental disabilities may be substituted for a degree at the rate of six months full time experience for each missing semester).
• One year of experience in the field of developmental disabilities.
