JOB SUMMARY
The Nurse Practitioner (NP) will play a vital role in delivering high-quality, patient-centered primary care services to PACE participants, both in the clinic and, when necessary, in their homes. As a key member of the interdisciplinary care team (IDT), the NP will serve as a liaison between participants, community physicians, and the care team, ensuring seamless coordination of care. This role is responsible for developing and implementing comprehensive, individualized care plans that align with participants' personal health goals, proactively addressing changes in their conditions, and facilitating preventative primary care services. The NP will have the opportunity to make a meaningful impact by enhancing the health, independence, and quality of life of the aging population.
KEY RESPONSIBILITIES
- Conducts comprehensive History and Physicals on new participants and current participants in any care setting (clinic, member's home in the community, nursing home, hospital, other care settings).
- Provides evaluation and treatment of participants during acute illness or the emergence of new complaints or symptoms.
- Determines the need for and facilitates referrals to other medical specialists when the participant's condition warrants.
- Evaluate the participants' plan of care and revise it as appropriate.
- Provides preventive health maintenance for participants, including immunizations, screenings, and monitoring of pertinent indicators.
- Prescribes and adjusts medications within the scope of prescriptive authority and current practice guidelines.
- Orders, reviews, and provides instruction to participants regarding therapeutic diet.
- Orders, facilitates, and reviews diagnostic examinations such as laboratory, radiology, etc.
- In collaboration with the Site Medical Director, actively participate in Infection Control and Surveillance activities.
EDUCATION & EXPERIENCE REQUIRED
- Graduate from an accredited school of Nursing.
- Current and valid registration to practice as a Family, Adult, or Geriatric Nurse Practitioner.
- AANP or ANCC Board Certification required.
- Valid and up-to-date DEA certificate is required.
- Basic Life Support certified by the American Heart Association or American Red Cross required
- Minimum two (2) years' experience working as an Adult, Family, or Geriatric NP, including one (1) year of experience working with frail elderly, preferably in a long-term care or primary medical care setting.
- Proven track record of strategic leadership, program expansion, and provider relations in a complex healthcare environment.
- Strong understanding of value-based care models, risk-based contracting, and regulatory compliance within Medicare/Medicaid frameworks.
- Excellent communication and presentation skills with diverse audiences.
- Strong analytical and strategic planning abilities with a track record of revenue growth and achieving business objectives.
