About

Team Select is looking for an RN Case Manager to join our Team Select family. Our case managers will oversee an assigned group of staff and patients. The Case Manager will report directly to the Director of Nursing. MUST HAVE VALID RN LICENSE.

What We Do:

  • The Case Manager will lead and/or assist in the orientation, training, supervision and management of Team Select nurses, including our New Grad nurses and manage assigned nursing cases.
  • The Case Manager would manage some Home Health Aide cases, including the training, supervision and management of the Home Health Aides on their assigned cases.

Case Manager Tasks:

  • Develop, deliver/implement, revise, and evaluate individualized patient-centered care plans under the supervision of a physician. All patient visits are scheduled and completed in strict accordance with the CMS Guidelines.
  • Provide case management to an assigned group of patients (Nursing and HHA mix)
  • Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs per state and payor requirements.
  • Act as an advocate for patient welfare and coordinates care between patients, their families/caregivers and/or their authorized representative, the agency and other healthcare providers/facilities/ outside agencies.
  • Attend case conferences and meetings as assigned. Most are pre-planned; some are not.
  • Understands and adheres to the Policy/Procedure, Best Practices, Infection Control Plan, EMS Plan, and the Compliance Plan. Be flexible - there are frequent changes in Home Care.
  • Timely documentation completion and submission is a requirement to maintain CMS/State compliance.
  • Maintain confidentiality of patient, employee, and agency matters at all times.
  • Takes on-call duty nights, weekends, and holidays, as assigned - shared with all Case Managers. (Fielding clinical calls - not covering shifts if there are call-outs. )
  • Time management, organization and attention to details is important to succeed, especially in a hybrid role. Basic computer skills is a must - all of our documentation is electronic, and we rely heavily on communication via Teams, e-mail, etc. You will receive a company laptop to use.
  • Maintain acceptable attendance status, per Agency policy, and be on time to patient visits, meetings, calls, etc.
  • Must be a team player. All members of the clinical and operations team work together to support one another. Our positive team culture and collaboration is a priority.
  • There is a lot of driving involved - reliable transportation is a must. There is mileage reimbursement.
  • Education is something that is highly valued at Team Select. It doesn't just include our HHAs, New Grads, and seasoned nurses. Our internal employees, both Ops and Clinical Team, receive the education needed to start in their positions and receive continuous support and training throughout their career with Team Select.

Job Requirements:

  • Minimum of 1 year nursing experience and a current RN license in the state of practice
  • Home Health Experience (Most beneficial, but not required)
  • Pediatric experience (Preferred, but not required - must want to work with children and adults)

Pay: $70, $75,000.00 per year
