Rehabilitation Physical Therapist (PT)

Location: Germantown, Tennessee
Assignment Dates: January 5, 2026 – April 4, 2026
Pay Rate: $45–$50 per hour

Position Summary

We are seeking a dedicated Rehabilitation Physical Therapist to join our healthcare team in Germantown, TN. In this role, you will evaluate, treat, and support patients in restoring mobility, reducing pain, and improving functional independence through evidence-based rehabilitation practices.

Key Responsibilities

  • Perform comprehensive patient evaluations to identify rehabilitation needs and functional limitations
  • Develop and implement individualized treatment plans focused on mobility, strength, balance, and pain management
  • Deliver therapeutic interventions including manual therapy, neuromuscular re-education, gait training, and functional mobility exercises
  • Treat patients recovering from orthopedic injuries, neurological conditions, post-surgical procedures, and chronic illnesses
  • Monitor patient progress and modify treatment plans to ensure optimal outcomes
  • Collaborate with interdisciplinary teams including Occupational Therapy, Speech Therapy, Nursing, and Care Management
  • Educate patients and caregivers on home exercise programs, injury prevention, adaptive techniques, and long-term wellness strategies
  • Maintain timely and accurate documentation in EMR systems in compliance with regulatory and facility standards
  • Apply sound clinical judgment supported by anatomy, physiology, and rehabilitation science

Qualifications

  • Active Tennessee Physical Therapist License
  • Minimum 2 years of rehabilitation experience (inpatient rehab, outpatient rehab, or post-acute preferred)
  • Strong knowledge of functional mobility, gait mechanics, and neuromuscular retraining
  • Experience treating orthopedic, post-surgical, geriatric, and neurological patient populations
  • Proficiency with EMR documentation
  • Excellent communication, clinical reasoning, and patient education skills
