Job Overview
We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will provide specialized therapeutic services to patients across various settings, including inpatient, outpatient, home health, schools, and skilled nursing facilities. This role involves developing and implementing personalized care plans to improve patients' functional abilities, assist with activities of daily living (ADLs), and support recovery and independence. The Occupational Therapist will collaborate with multidisciplinary teams to deliver high-quality patient care and promote optimal health outcomes.
Duties
- Assess patients' physical, cognitive, and emotional needs through comprehensive evaluations.
- Develop individualized treatment plans focusing on improving motor skills, sensory integration, and functional independence.
- Assist patients with ADLs such as dressing, grooming, feeding, and mobility to enhance their quality of life.
- Implement therapeutic interventions tailored for diverse populations including pediatrics, geriatrics, individuals with autism, disabilities, or post-acute conditions.
- Coordinate discharge planning and case management to ensure seamless transitions between care settings.
- Work closely with families, caregivers, and other healthcare professionals to optimize treatment outcomes.
- Document medical records and patient progress accurately to assigned EMR system while adhering to regulatory standards.
- Participate in interdisciplinary team meetings to review patient care plans and adjust therapies as needed.
- Provide education on injury prevention, ergonomics, and health maintenance related to occupational health or medical conditions.
- Support early intervention programs and work within school settings when applicable.
Qualifications
- Valid license or certification as an Occupational Therapist in the respective state or region.
- Experience working with diverse populations such as pediatrics, geriatrics, individuals with autism or disabilities, acute care patients, ICU experience, skilled nursing facilities, post-acute care, home health, outpatient clinics, or school-based settings is highly desirable.
- Strong knowledge of physiology, anatomy, medical terminology, and case management principles.
- Ability to develop effective care plans tailored to individual patient needs.
- Excellent communication skills for collaborating with multidisciplinary teams and educating patients and families.
- Compassionate approach with a commitment to improving patient outcomes through evidence-based practices.
