Citizens Against Domestic Violence, Inc Case Manager Job Description
The Case Manager works closely with the residents to empower them in meeting their goals. They collaborate with other key team members to ensure residents' needs are addressed and they coordinate within the community as needed.
Responsibilities for the Case Manager include, but are not limited to the following:
General:
Attend staff meetings, one-on-one meetings with the Shelter Manager and collaboration meetings and attends Residential Advocate meetings as needed.
Responding to calls from the Lake Area SART (Sexual Assault Response Team), as needed.
Complete both outreach and shelter intakes and providing new residents with orientation to assist in their transition to group living.
Work w/ Residential Advocates to coordinate shelter duties; including grocery shopping list and chore list.
Keeps Daily Update notes, when on site.
Completes shelter rounds as needed.
Works to maintain data integrity on Vela.
Works with Residential Advocate to update and maintain intake and outreach folder resources, as needed.
Assisting Shelter Manager by reporting maintenance issues; including new lighting, changing air filters, checking for bugs, ordering new mattress covers, ordering new bedding, and making sure residents have needed items to run a household.
Discuss exit procedures with clients when needed; supplying them with an exit survey.
Provide crisis line, office, and house coverage during the shift as needed. Assess immediate crisis needs of clients and address appropriately. Manage incoming calls and walk-ins and complete necessary documentation. Support the daily operations of the shelter office.
Specific:
Meets with each resident on a twice-per-week basis to create, review and revise goals.
Assisting residents with life-skills training such as nutrition, finding parenting classes, financial management, etc.
Referring clients for Crime Victim's Compensation procedures, if necessary, to the Legal Programs Coordinator or Shelter Manager.
Assisting clients w/ Safe at Home application, if needed.
Educating each resident regarding the programs and support groups available and why it's important to their progress in shelter.
Providing guidance to all residents for appropriate referrals to other social service agencies while working with the Coordinators to assist residents in reaching their goals.
Hold house meetings weekly in an effort to make sure shelter runs efficiently.
Has clients' complete surveys once per month.
Attends community meetings in regard to housing and transportation
Complete all documentation as required.
Provides transportation for clients when necessary.
Oversee shelter environment and activities, facilitate problem solving for in-house conflicts and crises, encourage and model appropriate shelter interactions, mediating when necessary.
Provide trauma informed interventions with clients.
Qualifications:
· Must demonstrate compassion, empathy, conflict management, decision quality, listening skills, problem solving, time management, multitasking, self-knowledge, and an understanding toward clients.
· Maintain confidentiality, security, and safety of clients.
· Familiarity either a strengths-based perspective and/or a working knowledge of trauma informed care.
· College degree preferred in related field or 2 – 4 years' experience working in human services field.
· Clear a Missouri Highway Patrol criminal background check and a Family Care Safety Registry background check.
· Working knowledge of DV/SA and/or willingness to learn. Must complete 40 hours of related training and shadowing before first solo shift.
· CPR Certificate and/or willingness to get and maintain one.
· Be able to work well with diverse colleagues and clients alike.
· Good oral and written communication and computer skills are required.
· Ability to read, write, converse, and understand English.
· Valid Missouri driver's license and car insurance
· Proficient use of MS Suite, Windows OS.
Physical Requirements:
Repetitive movement of hands and fingers – typing and/or writing. Occasional standing, walking, stooping, kneeling, or crouching. Reach with hands and arms. Talk and hear.
