Overview
We are seeking a compassionate, organized, and detail-oriented Client Services Coordinator to join our care team. In this essential role, you will serve as the central point of communication between residents, families, guardians, case managers, and internal staff. You will help ensure smooth service delivery, timely coordination of care, and a supportive, professional experience for everyone involved.

Key Responsibilities
- Serve as the primary point of contact for residents, families, guardians, case managers, and referral partners, responding to inquiries promptly and professionally
- Coordinate client intake, admissions, transitions, and ongoing service needs to ensure continuity of care
- Maintain accurate client records, service plans, and communication logs using electronic systems and internal documentation tools
- Support care teams by coordinating schedules, appointments, assessments, and follow-up communication
- Assist with onboarding new residents, including gathering required documents, coordinating assessments, and facilitating smooth transitions
- Communicate effectively with interdisciplinary teams to ensure care plans, staffing, and services align with client needs
- Monitor client feedback and concerns, escalating issues appropriately and helping resolve them in a timely, respectful manner
- Collaborate with leadership and administrative staff to support compliance with state regulations, licensing requirements, and payer expectations
- Build strong, trusting relationships with clients and their support networks to promote satisfaction and long-term engagement
Skills & Qualifications
- Strong organizational and coordination skills with the ability to manage multiple clients and priorities
- Experience in client services, care coordination, case management, or administrative roles within home care, assisted living, or healthcare settings preferred
- Familiarity with electronic health records (EHR), care management systems, or client tracking tools
- Understanding of home care, assisted living, or community-based services (Medicaid, waivers, county programs a plus)
- Excellent verbal and written communication skills with the ability to communicate clearly and compassionately
- Strong problem-solving skills and ability to remain calm and professional in sensitive or high-pressure situations
- Attention to detail and commitment to confidentiality and ethical standards
- Passion for delivering high-quality, person-centered care
