Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team, providing high-quality patient care across various settings. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in electronic health record (EHR) systems such as eClinicalWorks. This role offers an opportunity to work in a dynamic environment, including hospitals, long-term care facilities, assisted living communities, and outpatient clinics. The LPN will be responsible for delivering compassionate care while adhering to HIPAA regulations and maintaining accurate medical documentation.

Duties

  • Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories.
  • Administer medications and injections safely following established protocols.
  • Assist with diagnostic evaluations such as laboratory specimen collection, processing, and imaging procedures.
  • Document patient care accurately using EMR/EHR systems like Athenahealth or NCQA standards.
  • Educate patients and families on health coaching, medication management, and disease processes such as infectious diseases or chronic conditions.
  • Call back all diagnostic results, send in medications according to provider recommendations, establish great rapport with patients and families.

Qualifications

  • Valid Licensed Practical Nurse (LPN) license in the state of practice.
  • Proven experience with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards.
  • Experience working in diverse clinical environments including hospital medicine, emergency room, urgent care, outpatient clinics, or home & community care settings.
  • Proficiency with EMR/EHR systems such as eClinicalWorks.
  • Strong knowledge of physiology and anatomy relevant to patient assessment and diagnostic evaluation.
  • Ability to perform venipuncture, phlebotomy, suturing, sterile processing techniques, and basic life support (BLS).
  • Excellent communication skills with the ability to work effectively with patients of all ages—including children from neonatal to toddler care—and individuals with disabilities or developmental challenges.
  • Knowledge of infection control protocols and aseptic techniques necessary for safe practice in sterile environments like operating rooms or dialysis centers.

Pay: $ $22.00 per hour
