Summary of Duties:

Under the immediate direction of the Case Manger Supervisor/Therapist, the Case Manager will provide individual and family support counseling and group psycho education to at-risk children and adolescents and their families. Services will be based on a comprehensive approach and scope of work provided by the Mayor's GRYD office, to address family and client concurrent needs. The Case Manager will provide linkage between the families and identified community resources needed to assure that family members develop the necessary skills for self-empowerment and self-sufficiency.

EXAMPLES OF DUTIES AND RESPOSNABILITIES: Performs intake assessment on new clients and their families. Develop strength-based genograms on clients and their families. Develop Service Plans in conjunction with the family and Supervising Case Manager. Participate in strategic team meetings, including peer, individual, and family meetings. Provide intensive case management to families and clients to identify and address concurrent family needs. Perform home visits, consistently, to provide crisis prevention, intervention, and to provide problem-solving interventions for family dysfunctions. Lead in the development and implementation of therapeutic life skills, educational and recreational activities which will promote personal growth and family empowerment.

EXAMPLES OF DUTIES AND RESPOSNABILITIES: Must supervise and be interactive with youth during field trips and any other client activity. Provide community networking with allied professionals, including schools, "Great things are happening in Watts" juvenile court system, probation department, and public welfare agencies. Provide advocacy, referral, and linkages with community-based resources. Complete all requested internal and funding agency documentation in a timely manner. Will transport clients to and from designated areas, as assigned. Other responsibilities as assigned by the Project Director, Deputy Director, and/or Case Manager Supervisor.

QUALIFICATIONS AND EXPEREINCE: At least one year related experience working with children teens, and their families in a social service or mental health program. Minimum of 2 years' experience in Case Management or related field. A valid Class C Driver's License and personal liability insurance for a dependable vehicle is required.

Education: Bachelor's degrees from an accredited college; preferably in Social Work, Psychology, or Sociology or other related field is desirable. Experience may replace required degree.

Job Type: Full-time

Pay: $ $28.88 per hour

Expected hours: 40 per week
