What You’ll Do as a Home Health Physical Therapist: Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function. Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life—wherever they call home. Master’s degree in Physical Therapy from an accredited program. Current PT license in the state of employment (or eligibility to obtain).
