*Position Purpose*
The primary purpose of this position is to identify and meet the medical, physical, and psychosocial needs of each resident in accordance with physician orders, the plan of care, and all federal, state, and local regulations. The RN/LPN assumes the authority, responsibility, and accountability necessary to carry out duties within their nursing scope of practice while ensuring the highest quality of resident care at all times.

Paid out according to facility bonus schedule for eligible new hires.
*Job Requirements*
* Graduate of an accredited nursing program with an active, unrestricted RN or LPN license in the state of employment. Must maintain active licensure and certifications throughout employment.
* Previous nursing experience preferred; long-term care experience strongly desired.
* Must complete all facility new-hire requirements, including background check, Food Handler’s permit, BLS certification for healthcare providers, etc.
* Must complete new hire orientation and ongoing training/in-services as required.
* Excellent interpersonal communication skills and ability to work collaboratively with others.
* Strong organizational, critical thinking, decision-making, and time-management skills.
* Ability to communicate proficiently in English, both orally and in writing.
* Proficiency with computers and electronic medical record systems.
* Willingness to comply with all laws, regulations, and company policies, including dress code, attendance, and professional standards.
* Flexibility to work outside standard business hours, including nights, weekends, holidays, and on-call shifts.
* Professional participation in regulatory surveys and inspections.
* Ability to perform physical tasks required of nursing staff, including lifting/carrying 30+ pounds, or ability to be reasonably accommodated.
* Ability to cope with the emotional and mental stress inherent in caring for geriatric, dementia, mentally ill, and disabled populations.
* Ability to perform assigned tasks with minimal supervision and assist with other duties when needed.

*Summary of Principal Accountabilities*
The licensed nurse is responsible for all phases of direct resident care for assigned residents on their shift. Certain tasks may be delegated in accordance with the Nurse Practice Act, with appropriate supervision.
*Responsibilities Include:*
* Receive and provide shift report, including controlled substance count and reconciliation.
* Deliver person-centered care, honoring resident rights, preferences, and individualized care plans.
* Administer and document medications and treatments accurately, including refusals.
* Monitor resident response to treatments; notify providers of changes in condition.
* Enter and process physician orders in a timely manner.
* Order and maintain adequate medication and supply levels; keep carts clean and stocked.
* Communicate resident condition changes to the DON, provider, and resident representative as required.
* Complete incident reports and associated assessments and notifications.
* Review and update care plans based on resident needs and changes in condition.
* Complete assessments, progress notes, and documentation accurately and on schedule.
* Collect lab specimens, complete requisitions, and report results.
* Assist with admissions/readmissions, including assessments, orders, documentation, and resident orientation.
* Assist with resident discharges, including education, documentation, and medication preparation.
* Work collaboratively with other departments and contracted service providers.
* Help maintain clean and orderly work, storage, and break areas.
* Perform other duties as assigned by supervisor(s).
Job Type: Full-time
Pay: $30.54 - $36.77 per hour
Expected hours: 36 – 48 per week
