*Position Summary*
As a *Home Health Physical Therapist*, you will:
* Provide skilled physical therapy services in patients’ homes
* Assess functional abilities, mobility, strength, balance, and pain management needs
* Develop and implement individualized treatment plans based on physician orders and patient goals
* Monitor patient progress and adjust interventions as needed
* Collaborate with the care team to ensure high-quality, coordinated care
This position reports to the *Senior Clinical Manager* for the Boston area.
*Key Responsibilities*
* *Patient Evaluation:* Assess mobility, strength, balance, functional abilities, and safety needs
* *Treatment Plans:* Create and implement individualized care plans for each patient
* *Therapeutic Interventions:* Deliver exercises, gait training, balance activities, pain management, and other PT interventions
* *Patient & Caregiver Education:* Teach home exercise programs, safety techniques, and strategies for functional independence
* *Monitor & Adjust:* Continuously evaluate progress and modify treatment plans to meet goals
* *Documentation:* Maintain accurate records in compliance with agency policies and regulatory standards
* *Team Collaboration:* Communicate with physicians, nurses, OTs, care managers, and coordinators to provide integrated care
*Qualifications*
* Current Massachusetts *Physical Therapist (PT) license*
* Minimum *2 years of clinical experience*
* Valid driver’s license and reliable transportation
* *Home health experience preferred*
