About
We are seeking a dedicated and skilled *Per Diem Home Health Physical Therapist* to join our healthcare team. In this role, you will provide individualized physical therapy services to patients in their homes, helping them regain mobility, improve physical function, and enhance their quality of life. The ideal candidate will have a strong understanding of physical therapy practices, excellent communication skills, and the ability to work independently in a home health setting.
*Key Responsibilities*
* Conduct comprehensive in-home evaluations of patients to determine their physical therapy needs.
* Develop and implement personalized treatment plans focused on achieving patient-centered goals.
* Administer therapeutic interventions to enhance mobility, strength, balance, and overall physical function.
* Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
* Provide patient and family education on therapy goals, exercises, and home safety strategies.
* Collaborate with the broader healthcare team, including nurses, occupational therapists, and physicians, to ensure coordinated care.
* Document patient assessments, treatment plans, and progress notes accurately and in a timely manner, adhering to regulatory requirements.
* Participate in discharge planning, providing recommendations for continued care and equipment needs to ensure a smooth transition.
*Required Skills and Qualifications*
* Proficient in medical terminology relevant to physical therapy and home health care.
* Strong knowledge of physical therapy techniques, modalities, and best practices in a home health setting.
* Familiarity with discharge planning processes for effective transitions from therapy to independence.
* In-depth understanding of physiology, anatomy, and rehabilitation principles.
* Ability to adapt care plans to address diverse patient populations, including geriatrics.
* Self-motivated with excellent time management skills to manage a flexible per diem schedule.
* Exceptional communication skills for interaction with patients, families, and interdisciplinary teams.
*Additional Requirements*
* Valid Physical Therapy license in the state of practice.
* Current CPR certification.
* Reliable transportation to travel between patient homes.
* Experience in home health or acute care settings is preferred but not required.

Pay: $60.04 - $65.77 per hour
