*Job Overview*
We are seeking a dedicated and knowledgeable individual to join our team as a Clinical Pharmacist. This role is essential in providing high-quality pharmaceutical care to patients, ensuring optimal medication management, and supporting healthcare professionals in a clinical setting. The ideal candidate will possess a strong background in pharmacy practice, particularly in acute care and critical care environments, with a focus on patient service and safety.
*Duties*
* Conduct comprehensive patient assessments to determine medication needs and therapeutic outcomes.
* Provide clinical pharmacy services including medication therapy management, compounding medications, and IV infusion preparation.
* Collaborate with healthcare teams to develop and implement patient-specific treatment plans.
* Monitor patient responses to medications and adjust therapies as necessary.
* Maintain accurate inventory management of pharmaceuticals and ensure compliance with HIPAA regulations.
* Utilize pharmacy software, EMR systems, and EHR systems for documentation and patient care coordination.
* Educate patients on medication administration, immunizations, and pain management strategies.
* Participate in the training of pharmacy technicians and other staff members as needed.
* Ensure adherence to aseptic techniques during compounding processes.
*Qualifications*
* Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
* Completion of a pharmacy residency or equivalent clinical pharmacy experience preferred.
* Experience in LTC pharmacy, pediatrics, critical care, or hospital settings is highly desirable.
* Strong knowledge of physiology, anatomy, medical terminology, and pharmacology principles.
* Proficiency in IV insertion techniques and acute care practices.
* Excellent communication skills with a focus on patient service and education.
* Ability to work collaboratively within a multidisciplinary healthcare team.

Job Types: Full-time, Part-time
Pay: $50.00 - $60.00 per hour
