Stonebrook Post Acute is seeking a dedicated and skilled *Physical Therapist* to join our healthcare team. The ideal candidate will have a strong background in patient care in SNF care. This role involves developing and implementing personalized care plans to improve patient mobility, function, and overall quality of life.
*Duties*
* Assess patients’ physical conditions through comprehensive evaluations, including mobility, strength, and functional capacity.
* Develop and implement individualized *care plans* based on patient needs and treatment goals.
* Manage *discharge planning* to ensure smooth transition from hospital or clinic to home or community settings.
* Educate patients and their families on exercises, injury prevention, and health maintenance strategies.
* Document patient progress accurately using *PCC and NetHealth* and maintain detailed records of treatments provided.
* Stay current with advancements in *physiology knowledge*, *anatomy*, and medical terminology relevant to physical therapy practices.
* Participate in ongoing professional development to enhance skills in areas such as *orthopedics*, *geriatrics*, *pediatrics*, and specialized techniques like *trigger point therapy*.
*Experience*
* Proven experience working in SNF settings.
* Demonstrated ability to develop effective *care plans* for diverse populations such as pediatric, geriatric, orthopedic, or neurological patients.

Job Types: Full-time, Part-time, PRN
Pay: $55.00 - $75.00 per hour
