We are currently seeking an experienced Nurse Practitioner (NP) to provide virtual clinical oversight and interim primary care support for a defined community care area. This role is designed to be consultative, virtual-first, and sustainable, offering meaningful clinical work without the pressures of a traditional clinic or permanent roster.

About the Role

This position supports a small number of medically complex clients receiving structured home-based services. Many encounters are handled virtually, with in-person visits required only when clinically necessary and pre-approved.

Each Nurse Practitioner is assigned one service area, with coverage structured to be focused, predictable, and collaborative.

Each Nurse Practitioner supports a small cluster of neighboring communities within Ontario Health at Home's Central service area.

Examples include:

  • York Region & North Toronto corridors

The role is virtual-first, with in-person visits required only when clinically appropriate and within the assigned cluster.

This role is not clinic-based and not daily home visiting.

Requirements

Interim Primary Care Responsibility

Some clients supported through this role may be temporarily without a family physician.

In these situations, the Nurse Practitioner provides interim, time-limited primary care oversight, including assessment, medication management, and clinical decision-making, until long-term primary care attachment is secured.

This is not an open-ended or indefinite rostered role.
Care is delivered within a shared, interdisciplinary model and is tied to the duration of the home-care episode.

Key Responsibilities
  • Provide virtual clinical consultations (video, phone, and chart review)
  • Deliver interim primary care oversight for unattached clients when required
  • Review changes in condition and recommend next steps
  • Support care planning and escalation decisions
  • Collaborate closely with nursing and therapy teams
  • Conduct in-person visits only when clinically necessary and pre-approved

Benefits

Compensation & Mileage

We offer competitive, consult-based compensation that reflects senior clinical expertise:

  • $110–$140 per virtual consult (30–45 minutes)
  • $160–$200 per in-person visit, when required
  • $75/hour for any other duties (admin, etc)
  • Mileage reimbursed at competitive rate

Qualifications
  • Registered Nurse Practitioner in good standing with the CNO
  • Comfortable working at full NP scope
  • Strong experience with complex or medically fragile clients
  • Confident and thoughtful clinical decision-maker
  • Comfortable delivering care virtually
  • Values flexibility, autonomy, and collaborative practice
