As a Home Health Care Physical Therapist, you’ll help people move better, feel stronger, and get back to doing what they love. You’ll work one-on-one with patients to create personalized treatment plans, support them through their recovery, and celebrate their progress every step of the way. This role is all about building trust, offering encouragement, and making a real difference in people’s lives.
*Schedule*
● Full-Time
● Monday- Friday
*Key Responsibilities*
· Travel locally to provide in-home physical therapy to patients
· Evaluate patients and develop individualized treatment plans
· Deliver hands-on, evidence-based therapy to improve mobility and quality of life
· Educate patients and families on home programs and safety strategies
· Collaborate with nurses, physicians, and other care providers

*Qualifications*
● Licensed Physical Therapist (PT) in the state of Missouri
● Reliable transportation and valid driver’s license
● Strong clinical skills with a passion for helping others
● Excellent communication and organizational abilities
● Home health experience is a plus—but not required!
