*Duties*
* Dispense prescriptions accurately using advanced pharmacy software and electronic health record (EHR) systems while ensuring compliance with HIPAA regulations.
* Conduct thorough patient assessments to determine appropriate medication therapies, including evaluating medical histories, physiology, anatomy, and medication interactions.
* Provide expert clinical pharmacy services in specialized areas such as LTC (Long-Term Care), critical care, pediatrics, and acute care settings.
* Collaborate with healthcare teams to develop and implement medical management plans focused on pain management, immunizations, and medication administration.
* Compound medications with aseptic technique in sterile environments, including IV infusions and other parenteral therapies.
* Manage inventory levels efficiently to ensure the availability of essential medications while maintaining proper storage conditions.
* Perform patient care activities such as IV insertion, medication reconciliation, patient education on drug therapies, and assessing patient responses to treatments.
* Administer immunizations following CDC guidelines and maintain accurate documentation of all immunization activities.
* Educate patients on medication use, potential side effects, and adherence strategies to promote optimal health outcomes.
* Supervise pharmacy technicians by providing guidance on pharmacy operations, medication preparation, and inventory management tasks.
* Ensure compliance with all regulatory standards including HIPAA privacy rules and hospital policies related to pharmacy practice.
*Qualifications*
* Valid pharmacist license with state board certification; additional certifications in immunizations or specialized areas are a plus.
* Proven clinical pharmacy experience in hospital or acute care environments; experience in LTC or critical care is highly desirable.
* Strong knowledge of medical terminology, physiology, anatomy, and pharmacology principles related to medication therapy management.
* Experience with pharmacy software systems (such as EMR/EHR platforms) and inventory management tools.
* Demonstrated ability to perform compounding medications using aseptic techniques safely and accurately.
* Familiarity with IV infusion procedures, IV insertion techniques, and sterile compounding standards.
* Excellent patient service skills with the ability to assess patient needs effectively and communicate complex information clearly.
* Knowledge of medical management strategies including pain management protocols and pediatric medication considerations.
* Previous pharmacy residency or extensive clinical pharmacy experience preferred; experience in hospital or acute care settings is essential.
