*Position Summary*
CNS is seeking a compassionate Full-Time Physical Therapist to support Home Health patients in the greater SLC area. This role offers meaningful 1:1 care, greater autonomy, and the opportunity to work with patients needing care in home.
*Compensation & Benefits*
* *Per Visit Rates:* Starting at $65.00+ to $95.00+, We offer more hourly based on experience, education and work history.
* *Mileage:* $0.57/mile reimbursement

*Key Responsibilities*
* Provide skilled physical therapy services to Home Health patients in their homes
* Conduct comprehensive patient evaluations and develop individualized treatment plans
* Implement therapeutic exercises, manual therapy, gait training, and functional mobility interventions
* Educate patients, families, and caregivers on home exercise programs and safety modifications
* Coordinate care with physicians, nurses, occupational therapists, social workers, and interdisciplinary team members
* Monitor patient progress and adjust treatment plans as needed
* Maintain accurate and timely documentation per CMS, state, and CNS standards
* Adhere to safety, infection control, and HIPAA guidelines
*Qualifications*
* Current Utah PT license in good standing
* Graduate of an accredited physical therapy program (DPT preferred)
* CPR/BLS certification required
* Home health experience preferred; pediatric, geriatric, neurological, or orthopedic experience a plus
* Strong clinical assessment, critical thinking, and problem-solving skills
* Excellent communication and interpersonal skills
* Ability to work independently and manage time effectively with field-based caseload
* Valid driver's license, reliable transportation, and current auto insurance
*Physical Requirements*
* Ability to lift, transfer, and assist patients with mobility (up to 50 lbs frequently)
* Ability to drive throughout the SLC area and surrounding communities
* Ability to sit, stand, walk, bend, kneel, and squat for extended periods
* Ability to carry therapy equipment and supplies to patient homes

Pay: $40.00 - $95.00 per hour
