Job Overview
We are seeking a compassionate and motivated *Physical Therapist (PT)* to join our therapy team in a *nursing home / skilled nursing facility*. This position focuses on providing high-quality rehabilitative care to short-term rehab and long-term care residents, helping them improve mobility, strength, and overall functional independence.
Responsibilities
* Perform physical therapy evaluations and develop individualized treatment plans
* Provide skilled PT services in accordance with physician orders
* Treat residents with a variety of diagnoses including post-surgical, orthopedic, neurological, and general weakness
* Monitor patient progress and adjust treatment plans as needed
* Complete documentation accurately and in a timely manner (evaluations, daily notes, progress notes, discharges)
* Collaborate with nursing staff, physicians, and the interdisciplinary care team
* Educate residents and families on mobility, safety, and fall prevention
* Ensure compliance with facility policies and state and federal regulations
Qualifications
* Current *Physical Therapist license* in Arizona
* Graduate of an accredited Physical Therapy program
* Experience in a SNF or long-term care setting preferred (new graduates welcome to apply)
* Strong communication and time-management skills
* Ability to work independently and as part of a team

Pay: From $53.00 per hour
Expected hours: 10 – 40 per week
