We are currently seeking a *dedicated Full-Time Physical Therapist* to join our team.

*What You’ll Do:*
* Provide initial and ongoing assessments of patient needs, including OASIS assessments
* Develop and adjust treatment plans in collaboration with physicians
* Treat patients using physical means such as exercise
* Teach and guide patients and families in the physical therapy program
* Supervise Physical Therapy Assistants and paraprofessionals per agency policy
* Collaborate with the interdisciplinary care team
* Support discharge planning and complete PT discharge summaries
*What We’re Looking For:*
* Active *Florida PT License*
* Current *BLS CPR certification*
* 1+ year of experience in a home health or community setting
* AHCA Level II background screening clearance
* Familiarity with *WellSky/Kinnser EMR* preferred

Pay: $65.00 - $120.00 per hour
