We’re currently seeking *licensed * *Occupational Therapists (OTs)* to join our growing team of dedicated home health professionals across Colorado.
*Occupational Therapist (OT) – Job Duties:*
* Evaluate patients’ physical, cognitive, and psychosocial status to determine therapy needs in the home setting.
* Develop, implement, and revise individualized treatment plans and home exercise programs.
* Help patients restore function, improve mobility, and maintain independence with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal prep.
* Educate patients and caregivers on safety techniques and compensatory strategies for successful function at home.
* Recommend adaptive equipment and train patients/families on its proper use.
* Collaborate with interdisciplinary team including nursing, PT, speech therapy, and case managers.
* Document all care accurately and in a timely manner using electronic health record (EHR) systems.
* Ensure compliance with all regulatory and agency standards.
*Qualifications*
* Active *Colorado license* as a *OT*
* Graduate from an accredited Occupational Therapy program
* Minimum of 1 year clinical experience preferred (home health experience a plus)
* Reliable transportation and valid driver’s license
* Strong communication, clinical reasoning, and time management skills
* Ability to work independently in a home setting
