*Who We're Looking For:*
* Licensed Physical Therapist in Colorado (active and in good standing)
* Graduate of a program accredited by the National Board for Certification in Physical Therapy
* BLS/CPR certified
* Comfortable using tablets and electronic documentation (we’ll support you!)
* Reliable vehicle and valid driver’s license
*What You’ll Do:*
* Conduct in-home assessments and create customized plans of care
* Provide direct treatment and re-evaluations in coordination with the clinical team
* Document visits and communicate patient progress using our digital platform
* Educate patients and families on safety, mobility, and home exercise programs
* Be a key part of an integrated, compassionate care team
