*Landmark Home Health is looking for Physical Therapists in Monroeville, East Liberty, Penn Hills, Murrysville, Trafford, and surrounding areas*. As a Physical Therapist, you will provide high-quality care to patients in the comfort of their own homes. You will play a crucial role in helping patients regain their mobility and independence, improve their quality of life, and promote overall well-being. Strong clinical skills, computer skills, and of course Home Health Care experience is not required but a PLUS!
*We support the Public Service Loan Forgiveness (PSLF). The PSLF program helps individuals in public service roles alleviate the burden of student loans.*

*RESPONSIBILITIES:*
* Assists the physician in evaluating and assessing the level of function of the patient and in developing a Plan of Care.
* Interprets findings to the physician, patient, family and the professional nurse.
* Assists in establishing rehabilitative plans and goals to reach the patients optimum functional level.
* Provides comprehensive physical therapy treatment utilizing rehabilitation procedures to improve the patient's level of functioning.
* Advises and consults with patient's family and the professional nurse in implementing and evaluating the Plan of Care.
* Revises the treatment plan as indicated, upgrades program based on patient's progress and participates in discharge planning.
* Provides supportive care to the patient, family and significant others and assists them to accept and adjust to realistic plans and goals based on the patient's physical limitations.
* Assists with the selection and/or modification of equipment, appliances and physical set up of the home to meet the patient's needs.
* Recognizes the teaching needs of the patient, family and significant others. Develops a teaching plan to meet those needs.
* Initiates interdisciplinary communication to assure the effective implementation of comprehensive treatment. Participates in periodic patient evaluation conferences.
* Prepares clinical progress notes and completes required documentation promptly, accurately, and concisely in accordance with Agency Policy, Medicare and other third party reimbursement regulations.
*QUALIFICATIONS:*
* Current Pennsylvania Physical Therapy License is required
* Must have two years of current therapy experience
* Valid current driver's license and auto insurance coverage is required
*WORKING CONDITIONS:*
* Normal working hours are Monday through Friday, 8:00 am - 4:30 pm
* Participates in rotating weekend assignment rotation.
