HealthRIGHT 360's Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients-on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city's most underserved populations.

The Advance Practice Provider (APP) will deliver comprehensive healthcare services to patients experiencing homelessness in San Francisco, either from a mobile medical van or via Street Medicine (backpack medicine). The APP may also provide medical services at HR360's brick-and-mortar clinic, the Integrated Care Center (ICC) at 1563 Mission Street. The APP will have the ability to be creative, contribute to the formation and development of the team, and shape what Street Medicine looks like in this innovative setting. As part of a multi-disciplinary team, the APP will perform diagnostic, therapeutic, preventative, and health maintenance services.

KEY RESPONSIBILITIES

Clinical Care Responsibilities:

Provides comprehensive care, within the scope of credentialed privileges and standardized procedures, to HR360 patients in a team-based delivery model in the mobile medical van, on the street, in a homeless shelter, or in a brick-and-mortar clinic setting. Conducts street-based and mobile health visits to assess and treat patients with acute, chronic, and preventative health needs. Performs clinical assessments and procedures including wound care, point-of-care testing, immunizations, and injections. Facilitates linkages to specialty care. Documents care promptly according to charting protocols, applicable California state licensing requirements, and clinic policies in the electronic medical record (EMR). Completes patient sessions in a timely manner in accordance with schedule. Ensures appropriate follow-ups for all aspects of ordered care (e.g., specialty referrals, laboratory results, radiology reports, etc.). Responds to patient requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner. Advocates for patients experiencing homelessness, ensuring they receive fair and comprehensive healthcare services, and represent their needs within the broader healthcare system and with social service agencies. Provides case management services when needed by coordinating care, facilitating access to resources, and assisting patients in navigating the healthcare system, ideally in collaboration with the rest of the healthcare team. Works collaboratively with mental health staff to facilitate integration between primary care and mental health to improve patient outcomes. Provides on-call after-hours availability on a rotating basis. Obtains and maintains licensure, DEA registration and BCLS/CPR certification. Attends training and conferences related to the Street Medicine field, as directed by Mobile Medicine and Street Medicine director. Attend continuing professional education programs as required for certification/licensure. Performance and Quality Improvement Responsibilities:

Meets benchmarks for applicable quality indicators as noted in the annual quality management plan including the development of an appropriately sized continuity panel of patients and the maintenance of current productivity standards. Along with colleagues, assists in continuous quality improvement through ongoing problem/needs identification and problem-solving. Acts as change champion or lead in improvement activities. Organizational Responsibilities:

May supervise students and other learners as appropriate. Maintains availability for various administrative/clinical duties as requested by the supervisor (e.g., case presentations, in-service training to staff). Represents HealthRIGHT 360 at and/or attends professional conferences, in-service training, and meetings at the request or with approval of supervisor. Complete all mandatory training courses on an annual basis. QUALIFICATIONS:

Education, Credentials, and Experience

Graduation from an accredited school of nursing with a master's degree OR from an accredited physician assistant/associate program with a master's degree. Possession of a valid California license as an ANP or FNP or PA. Possession of national board certification as appropriate. First Aid & CPR certification. A minimum of 2 years' experience (preferably in ambulatory care, community health, or outreach settings). DEA Controlled Substances Registration Certificate. Valid California driver's license and access to registered and insured transportation. Desired:

3+ years' experience working with patients experiencing homelessness, ideally in a Mobile Medicine or Street Medicine setting. Strong background in Primary Care, Addiction Medicine, and Psychiatry. Experience working in ambulatory care and/or a community health center. Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and culture backgrounds and economic status. Experience working successfully with issues of homelessness, substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency. Knowledge, Skills and Abilities

Strong clinical skills in diagnosing and treating a wide range of medical conditions. Flexibility and creativity in providing care in non-traditional settings. Comfort working on teams and with patients experiencing homelessness. Knowledge of harm reduction, trauma-informed care, and anti-racist practices. LGBTQ+ Cultural competence and ability to work with a diverse population. Strong proficiency with Microsoft Office applications, specifically Word Outlook and Internet applications. Knowledge of co-occurring disorders and trauma-informed care. Experience in wound care and Phlebotomy is preferred. Preferred experience in giving long-acting injectable anti-psychotics, long-acting injectable buprenorphine. Ability to perform advanced ambulatory procedures: IUD placement, long-acting subdermal contraceptive implant placement, joint steroid injection, skin punch and shave biopsies, endometrial biopsy, and colposcopy.
