RN Coordinator - Infusion at Providence Health and Services summary: The RN Coordinator - Infusion is responsible for planning and coordinating new patient referrals and providing phone triage for existing patients, mainly focusing on infusion therapy in home, clinic, or alternate living environments. This role involves collaboration with healthcare providers, patients, caregivers, physicians, and payors within Oregon and southwest Washington. Candidates must have a valid RN license, home infusion nursing experience, and may provide direct nursing and case management services onsite. RN Coordinator- Providence Home Services- Oregon Schedule: Full Time/Days RN Coordinator role consists of the planning and coordination of new referrals. The position will also provide phone triage of existing patients. The majority of the responsibilities will be carried out onsite in office. May provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and/or caregiver, the physician and the payor.
