Care Case Manager Leader

Our organization is seeking a highly skilled and experienced Care Manager Supervisor to join our team. This role will be responsible for managing the initial review, triage, and assignment of patients to case managers, as well as coordinating all systems and services required for an organized, multidisciplinary, patient-centered care approach.

The successful candidate will ensure quality, cost-effective care for the identified patient population, supervise staff in the department, and act as a resource and role model for staff members. The incumbent will also function as a contact person for patients, families, healthcare team members, community resources, and employees, ensuring adherence to hospitals and departmental policies and procedures.

Key Responsibilities:

• Manage the initial review, triage, and assignment of patients to case managers
• Coordinate all systems and services required for an organized, multidisciplinary, patient-centered care approach
• Ensure quality, cost-effective care for the identified patient population
• Supervise staff in the department
• Act as a resource and role model for staff members

Requirements:

• 3 years of directly related experience
• Bachelor's degree in a related field (Master's degree preferred)
• Essential qualifications include RN licensure in NM or reciprocal agreement with NM, CPR for Healthcare/BLS Provider or Professional Rescuer within 30 days
• Physical conditions require light work exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or negligible amounts of force constantly

Working Conditions:

• Significant hazard includes chemicals and bio-hazardous materials requiring PPE
• Subject to annual contract and performance appraisal
• Tuberculosis testing completed upon hire and additionally as required
