About
We are seeking a dedicated and skilled Home Health Physical Therapist to join our healthcare team. In this role, you will provide comprehensive therapy services to patients in their home and community settings, supporting recovery, mobility, and independence. The ideal candidate will possess a strong foundation in physiology, anatomy, and medical terminology. Your expertise will contribute significantly to patient care plans, discharge planning, and rehabilitation efforts across diverse populations including pediatrics, geriatrics, and orthopedics.
This position has an average salary of $90k-$130k however you may get paid up to $100 an hour for certain tasks/services. Your actual salary will depend on your experience and how many patients you see weekly. This is a fulltime permanent position with a good benefit package.
Key Responsibilities:
* Conduct thorough assessments of patients’ physical conditions, mobility, and functional abilities within home and community settings
* Develop individualized care plans that align with medical prescriptions and patient goals
* Deliver therapy interventions such as therapeutic exercises, trigger point therapy, and manual techniques to improve strength, flexibility, and pain management
* Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transition of care
* Educate patients and caregivers on injury prevention, activity modifications, and home exercise programs
* Monitor patient progress regularly and adjust treatment strategies accordingly
* Document all patient interactions accurately within electronic medical record (EMR) systems
* Support outpatient clinics or inpatient units as needed to maintain continuity of care
* Participate in ongoing professional development to stay current with therapy best practices
*Requirements*
* Valid licensure as a licensed Physical Therapist
* Experience in home health care, community-based therapy services or healthcare experience.
* Strong knowledge of physiology, anatomy, and medical terminology relevant to rehabilitation
