Overview: Transform Lives as a Home Care Physical Therapy Assistant!

Are you a dedicated and compassionate Physical Therapy Assistant with a passion for helping patients regain their independence? We are looking for an exceptional  Home Care PTA  to join our team and make a real difference in the lives of those recovering from illness and injury.

Work primarily weekdays with a weekend rotation monthly. Enjoy the autonomy of managing your daily work schedule

About the Role:

As a Home Care Physical Therapy Assistant, you will:

  • Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.
  • Under the direction of a physical therapist, conduct treatment programs based on each patient's plan of care.
  • Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.
Responsibilities:
  • Physical Therapy Skilled Treatments.
  • Client Care Coordination.
  • Teaching, Supervision and Counseling Clients and Caregivers.
  • Documentation of Interdisciplinary Communication.
  • Problem Solving and Critical Thinking Skills.
  • Documentation.
  • Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
  • Time Management 
  • Assists in Deliver of Plan of Care
  • Able to recognize the need for a change in the plan of care and effectively communicating this to the supervising therapist in a timely manner
Qualifications:

Ability to assist in patient lifts, transfers, and guarded functional activity training.

Competencies Skills:

Essential:
* Clear Communication Skills Both Written and Verbal
* Able To Keep Confidential Information Regarding Patients, Team Members
* Able To Withstand Crisis Situations
* Has Skills to Provides Customer Service to Patients, Team Members and Visitors
* Proof Of Current/Valid Car Insurance
* Knowledge And Experience with Electronic Health Records
* Experience With Excel, Power Point, Word, Visio, Etc.
* Valid DL With No More Than 3 Moving Violations

Credentials:

Essential:
* PTA - Physical Therapy Assistant
* BLS - Basic Life Saving certification

Education:

Minimum required: Associates Degree

Other Information: Entry: USD $24.62/Hr. Max: USD $38.16/Hr.
