- Location: Arlington Heights, IL
- Full Time, 40 hours/week
- Hours: 8am-4:30pm, Monday - Friday

What you will do:
Reporting to the Service Line or specialty program leader (Cardiovascular, Neuroscience, Cancer, GI, Orthopedic, Geriatric), the Patient Navigator serves as the primary point of contact for the patient through the continuum of care; diagnosis, planning, treatment, pre and post-operative care, discharge and follow up. The Patient Navigator functions as a resource to patients, seeking patient input into the plan of care. The Patient Navigator engages and collaborates with all appropriate care givers (physicians and the entire care team) in the plan of care, while serving as the advocate for the patient. The Patient Navigator provides patient and family education, and access to hospital and community resources as well as community education and screening.
- Conduct comprehensive assessments of patients' medical, social, and psychological needs.
- Collaborate with healthcare providers to develop individualized care plans that address patients' unique needs and goals.
- Identify potential barriers to care and work to address them proactively.
- Serve as a liaison between patients, families, healthcare providers, and community resources to ensure a smooth transition of care.
- Monitor and manage patients' progress throughout their healthcare journey.
What you will need:
- Bachelors Degree Nursing or Masters Degree Nursing required
- Inpatient nursing for at least (1) year
- Experience working with neurology or epilepsy patient populations, preferred
- Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. required
