Job Description: Reporting to the Manager and in partnership with local Network Leaders, the Assessor Coordinator - Degree - Senior is responsible for education, implementation, clinical support and evaluation of home care services provided by interdisciplinary care teams. The incumbent will have a provincial role in consultation, development, and standardization of home care programming across the SHA. The incumbent is the clinical/program resource for all Home Care programming needs. They collaborate with other portfolios to support Primary Health Care/Health Service Managers ensuring front line staff have resources, training, and clinical support required to work consistently, safely, effectively, aligning with current best practice standards. They are the content expert, having advanced knowledge of programming, policy, guidelines, standards and best practices. They will advise on, develop and implement program changes.

Experience
- Two (2) years experience working in Home Care
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Bachelor of Physical Therapy is acceptable
- Bachelor of Social Work is acceptable
- Bachelor of Occupational Therapy is acceptable

- Registered/eligible for registration with the appropriate licensing body
