*Job Overview*
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy and patient care, with experience in various pharmacy settings, including long-term care (LTC) and acute care environments. This role is crucial in ensuring the safe and effective use of medications, providing exceptional patient service, and collaborating with healthcare professionals to optimize patient outcomes.
*Duties*
* Review and interpret medication orders to ensure accuracy and appropriateness.
* Provide patient assessments and counseling on medication therapy management.
* Administer immunizations and oversee medication administration processes.
* Manage inventory effectively, ensuring the availability of necessary medications.
* Utilize pharmacy software and electronic health record (EHR) systems for accurate documentation.
* Collaborate with healthcare teams to develop medical management plans for patients.
* Maintain compliance with HIPAA regulations and ensure patient confidentiality.
* Provide education on medical terminology, physiology, and anatomy to patients and staff.
*Qualifications*
* Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
* Pharmacy residency or equivalent clinical pharmacy experience preferred.
* Experience in LTC pharmacy, critical care, pediatrics, or hospital settings is advantageous.
* Proficient in using EMR systems and pharmacy software for efficient workflow management.
* Strong understanding of medication management principles, including compounding and administration techniques.
* Excellent communication skills with a focus on patient service and care.
* Ability to work collaboratively within a multidisciplinary team environment.
* Knowledge of pain management protocols and acute care practices is beneficial.

Pay: $59.53 - $76.69 per hour
