*Responsibilities*
* Provide comprehensive patient care through personalized physical therapy plans tailored to individual needs.
* Assess and document patient progress using EMR systems, ensuring accurate medical records.
* Collaborate with healthcare teams for effective discharge planning and care coordination.
* Educate patients and families on rehabilitation exercises and home care strategies.
* Utilize knowledge of anatomy and physiology to implement appropriate therapeutic techniques.
*Qualifications*
* Valid physical therapy license in the Commonwealth of Pennsylvania
* Strong knowledge of anatomy and physiology
* Proficiency in patient care and medical terminology
* Experience in discharge planning and developing care plans
* Familiarity with EMR systems and home health settings
* Background in geriatrics, and orthopedics
* Ability to work in acute care, inpatient, outpatient, and clinic environments
* Willingness to travel to patient homes.
* Excellent communication skills
* Remain professional at all times.
